No, we don’t accept walk-in visits at this time.

FOREIGN TRAVEL

We will schedule an office visit to address your child’s special travel needs. When making these appointments, it is important to be specific about the place of travel and the duration of the time you will be away. To this point: do not say, “We are going to South America for about two months.” A more precise response would be, “We are going to Peru for 6 weeks this summer.” By being specific, we will be certain to have the specific vaccines and medications necessary for your specific travel.

HEALTH FORMS

The office will gladly complete necessary health forms for your child. Please allow 7 business days for the completion of these forms. Your child’s annual WELL CHILD VISIT must be current before the form can be completed. Because of HIPPA regulations, these forms cannot be faxed or emailed, nor can they be left outside the office for pick-up. You may leave a stamped, self-addressed envelope and the form will be mailed. If you choose, you may come back to the office to get your completed form. Feel free to make a copy of your child’s form as we do not keep copies. Please be certain that you have completed the parents’ portion of the form and sign where indicated before leaving it with us. Failure to complete the parents’ portion will result in a delay in the completion. There is a $15 fee for the completion of all forms which must be paid at the time the form is presented for completion.

                                        LETTERS

Requests for letters of any kind require 10-14 business days for completion. It is the parents’ responsibility to provide in writing the nature of the letter requested, the name and address of the person receiving the letter, and any supplemental documentation as needed. There is a $25 fee payable at the time the letter is requested.

LATENESS AND MISSED APPOINTMENTS

We believe in empowering our patients and parents to take responsibility of and guard your own health. As a courtesy, you will receive a reminder call of your child’s appointment 1-2 days prior to remind you to confirm the date and time. However, we expect you to take responsibility for any appointments you make with the office. If you are late, we will do our best to accommodate you; however, you may need to reschedule your appointment.

ON-CALL SERVICE STARTS at 8:30 pm – 7 am M-F, Weekends /Holidays

When the office is closed for weekends, holidays, or inclement weather, we will always have a provider on call. You may reach us by calling the main office number. If indicated, a virtual visit may be recommended by the on -call physician. Otherwise, if you need to speak with the doctor after hours, you will be charged a $20 administrative fee.

Please reserve this service for urgent medical concerns ONLY. Do not call for a medical emergency; call 911 for ALL medical emergencies. Do not use this service for prescription refills, referrals, lab results, or appointments. These services will not be addressed after hours and will have to wait until the following business day. Prescriptions are refilled at regular FOLLOW-UP VISITS or Virtual Follow Up Visit Monday to Friday, not afterhours or on-call hours. The practice does not phone in prescriptions unless the patient is seen and our EMR is not communicating the information to your pharmacy.

We accept the following commercial insurances? Aetna, Cigna, Blue Cross Blue shield, Care first, Blue Choice, United Health Care. We accept these forms of insurance for primary care pediatrics only.  For Ear piercing, all Holistic Wellness services, Sprout program, mindful weight management are self-pay services.
